Once you have completed the online registration forms, please call Central Office at 630-463-3000 to schedule an appointment to provide the neccessary documents (residency, birth certificates, and identification) to the school district.
Once you have completed these two steps, please print the registration packet, complete the forms and return them to Mill Creek School.
For any student transferring from out of state, a new physical, dental, and vision exam is required in addition to a copy of immunizations. The physical exam needs to be completed in compliance with Illinois standards and dated no more than one year prior to the date of enrollment.
For any student transferring to our district from within the state of Illinois, a copy of his or her Kindergarten physical, dental, and vision exam and immunizations is required. If these copies cannot be provided, new exams will need to be completed and submitted. Please also note, in state transfer students are required to comply with the physical examination and immunization requirements of the grade they are entering. New dental exams are required for all second grade students.
