Student Registration:
To register your child for this FREE, voluntary screening, please go to https://screenings.yh4l.org/YH_enroll.aspx?echo=false or scan the QR code below. Click on the button at the bottom of the page that says, “I Give My Child Permission.”  You will need your student's school ID number to register.  NOTE: Each EKG is done privately in individual stalls separated by gender in the gym with females on one side and males on the other. Students will NOT have to remove any clothing (electrodes are placed under the shirt).
