GHS PE Medical Excusal Procedure
(Updated January 2025)

Students who are unable to participate in PE due to a medical issue must follow the procedures below:
-Submit a medical note to the nurse's office:
-Medical notes must be completed & signed by the healthcare provider stating:
  • Extent of the injury/illness
  • Specific limitations
  • Next follow-up visit
  • AND expected duration of excusal

-The nurses office will process the medical documentation:
-MEDICALLY-EXCUSED students will be placed in a study hall for the duration of the medical excusal. Assignments will be communicated/provided to students by the supervising PE teacher. It is the student’s responsibility to complete/submit these assignments to earn PE credit.
- MODIFIED ACTIVITY: for students requiring modified activity, PE classwork and activities will be modified according to healthcare provider orders. Modifications will be communicated to PE teachers by the nursing office.

DURATION OF MEDICAL EXEMPTION/MODIFICATION:
To ensure follow-up care during periods of PE medical excusal & modified activity from PE:
-Updated notes from any follow-up visit(s) must be submitted to the nursing office. Updated medical notes should be provided to nursing after each follow-up visit, until a final medical clearance is received. Notes should be submitted to nursing in a timely matter.
-If specific limitation(s) need to be extended past the date provided, an additional medical note will need to be provided by the healthcare provider.
-Nursing will follow up with parent/guardian at 4 weeks (i.e. monthly) if medical updates have not been otherwise provided
-Any medical documentation without an end date provided (i.e. “until further notice”), will default to 6 weeks from the start date. At 6 weeks, if updated medical documentation is not received by nursing, student will begin to earn zero credit for PE until an updated medical note is received.
-Students requiring medical excusal from PE, will also be excused/exempt from participation in school-sponsored sports activities for the duration of the PE medical excusal.

RETURNING TO PE:
-Students may return to full participation once a medical clearance note, signed by the managing healthcare provider, is received by nursing. Students are responsible for reporting back to PE on the specified medical release date.
-If modified activity is no longer required, this must be documented in the medical clearance note, including the date of release to full activity.
